Here are some very different resources along these lines:
- Very positive: http://www.humanrightstools.org/index.htm (Seems practical, open-ended, and useful to all kinds of different communities.)
- Medium: http://pioneersofchange.net/services/CoP%20Handbook2.pdf (Has inspiring drawings, but most of the ideas are based on reading, not experience)
- Medium: http://www.odi.org.uk/Rapid/Publications/Tools_KM.html (Broad coverage, so gives a lot of context.)
- I've seen some terrible toolkits -- not just because they are toolkits, but because they collect words, not experience, and put them together in a way that claims expertise where there is none.
